The Massachusetts Department of Public Health breaks the list of serious reportable events into six sections, with each section further divided into subsections as detailed below:
- Surgical Events
- Surgery wrong body part
- Wrong patient surgey
- Wrong surgical procedure
- Retained foreign object
- Death <24 hours ASA 1 patient
- Product or Device
- Contaminated drugs or device
- Device malfunction
- Air Embolism
- Patient Protection
- Infant discharged to wrong person
- Elopement
- Suicide/Suicide attempt
- Care Management Events
- Medication error
- Transfusion error
- Maternal death/disability
- Hypoglycemia
- Hyperbilirubinemia in neonate
- Stage 3 or 4 pressure ulcer
- Spinal manipulation
- Artificial insemination error
- Environmental Events
- Electric shock
- Oxygen or gas error
- Burn
- Fall
- Restraint/bedrails
- Criminal Events
