Hospital Mistakes Virtually Unchanged

The Massachusetts Department of Public Health breaks the list of serious reportable events into six sections, with each section further divided into subsections as detailed below:

  1. Surgical Events
    1. Surgery wrong body part
    2. Wrong patient surgey
    3. Wrong surgical procedure
    4. Retained foreign object
    5. Death <24 hours ASA 1 patient
  2. Product or Device
    1. Contaminated drugs or device
    2. Device malfunction
    3. Air Embolism
  3. Patient Protection
    1. Infant discharged to wrong person
    2. Elopement
    3. Suicide/Suicide attempt
  4. Care Management Events
    1. Medication error
    2. Transfusion error
    3. Maternal death/disability
    4. Hypoglycemia
    5. Hyperbilirubinemia in neonate
    6. Stage 3 or 4 pressure ulcer
    7. Spinal manipulation
    8. Artificial insemination error
  5. Environmental Events
    1. Electric shock
    2. Oxygen or gas error
    3. Burn
    4. Fall
    5. Restraint/bedrails
  6. Criminal Events
    1. Impersonation of health professional
    2. Abduction
    3. Sexual assault
    4. Physical Assault
